Bi-State's Board of Directors welcomes Carlos G Otis Health Care Center, Inc, dba Grace Cottage Family Health as a new member of Bi-State Primary Care Association. Grace Cottage's primary care team of physicians, physician assistants, nurse practitioners, nurses, and staff provide preventive and primary care and pharmacy services. Grace Cottage Family Health was recently awarded Level 3 Patient Centered Medical Home recognition and offers a range of services, including medical and mental health care, prevention and wellness services, acute care, and chronic care.
National Association of Community Health Centers
We are pleased to be able to do our part by providing quality health care to a rapidly growing number of veterans across the country. Every day, more and more veterans are getting their care at Community Health Centers. Health centers serve more than 300,000 veterans and their ranks have grown by 50 percent since 2008. With passage of the Veterans Choice Act in 2014, the National Association of Community Health Centers developed and implemented strategies designed to increase participation in the Veterans Choice Program. This is part of a broader national strategy to improve how veterans can access care right in their own communities through health centers and other local providers outside of VA facilities.
In September 2017, Bi-State was awarded its third Navigator Grant from the Centers for Medicare and Medicaid. The grant supports education and enrollment assistance for the public in the New Hampshire Health Insurance Marketplace.
Bi-State’s navigator, Marilyn Sullivan, M.Ed., will continue to lead the program and coordinate outreach activities with Community Health Centers statewide.
For more than 50 years, Community Health Centers have earned bipartisan Congressional support for consistently delivering cost effective and affordable quality health care nationwide. The Federally Qualified Health Centers in New Hampshire and Vermont offer comprehensive primary health care to address medical, oral, mental and behavioral health care needs as well as substance use treatment and recovery programs.
