Bi-State Primary Care Association honored Phil Lawson, MD and Steve Noyes, MSW, LADC, LISW — both of Ammonoosuc Community Health Services — with the 2007 New Hampshire Outstanding Clinician Award on May 23, 2007 at its annual meeting.
“Working as a professional team, Mr. Noyes and Dr. Lawson have identified and addressed the behavioral health needs of their primary care patients through an integrated assessment and care delivery model that challenges the traditional fragmenting of physical and behavioral medicine,” states Tess Kuenning, Bi-State executive director.
Lawson and Noyes have led the primary care team at Ammonoosuc Community Health Services in putting the patient at the center of the care model and bringing the practitioner to the patient. This approach has proven effective in reducing barriers to access for the patient population. Furthermore, they have been recognized regionally and nationally for their model of care and successful outcomes — a tribute to the delivery of comprehensive, patient-centered care.

Bi-State Primary Care Association honored John Brooklyn, MD with the 2007 Outstanding Clinician Award on May 23, 2007 at its annual meeting.
“Dr. Brooklyn has been a staff physician at the Community Health Center of Burlington (CHCB) for fourteen years,” states Joe Arioli, CHCB executive director. “Two of those fourteen years, Dr Brooklyn took on the role of interim Medical Director and provided invaluable leadership during those years of transition,” Arioli continues.
“Dr. Brooklyn was one of six from CHCB honored with the “Teacher of the Year” award by Fletcher Allen Health Care's Family Medicine Residency in 2006,” states Arioli.
“Dr. Brooklyn is an outstanding clinician not only because he is a highly regarded primary care physician devoting his career to the medically underserved, but also because of his groundbreaking work as an addictive medicine specialist.”
“John’s expertise is very much in demand in Vermont; he generously shares his knowledge, unique expertise and time as a speaker, and has lectured to medical staffs at various hospitals in Vermont,” declares Arioli.
